Great News! The dentist will be coming to your child’s school soon!

Important: Our dental team will require that all children positively identify

themselves by answering any two of the following questions:

• Name

• Date of birth

• Phone number

• Address (street only is fine)

• Parent or guardian name

If you have concerns that your child will not be able to independently identify

themselves, you can help to ensure that they are seen in a few ways:

o Make sure your child knows their name and parent or guardian name

in advance of the clinic (or any of the 2 identifiers listed above.)

o A parent or guardian can be present to personally identify the child

for the team.

o Provide a tag with a picture of your child along with identifying

information. Make sure your child is wearing the tag.

In this way, we ensure that we are providing care to the child who has a signed

consent form, with the correct medical history. This is essential for the safety and

well-being of all students at school.
